The HIPAA-compliant 835 4010 Electronic Remittance Advice is now available for hospitals, health care professionals and clearinghouses. This replaces the 3051 version.

Hospitals and health care professionals who wish to receive the 835 transaction directly should complete the ERA Addendum - Corporate Headquarters form. Clearinghouses must complete the ERA Addendum - Billing Services and Clearinghouse form.
