Get Paid Faster with EFT

Are you ready to stop going to the bank to deposit checks? Would you like to receive reimbursement faster? If so, then electronic funds transfer (EFT) is for you!

EFT is available to all providers, hospitals and professionals included, both in network and out of network. If you would like to participate in the EFT program, complete the following two forms and mail them to:
