Claim Submission Formats and Filing Assistance

We currently accept the following claim submission formats:

  • CMS-1500 claims (filed via the Web or paper)
  • CMS-1500 claims filed via the Superbill tool in My Insurance Manager
  • CMS UB-04 claims (filed via the Web or paper)
  • HIPAA 4010A1 electronic format claims (professional and institutional)


Web Claims/My Insurance Manager

You can file both CMS-1500 and CMS UB-04 claims to us via the Web using My Insurance Manager. This feature allows you to submit claims for all BlueCross BlueShield of South Carolina (including dental), State Health Plan, Federal Employee Program (FEP), Planned Administrators, Inc. (PAI), Employee Benefit Services (EBS) and Thomas Cooper members.

On behalf of BlueCross, PAI, EBS and Thomas Cooper provide third party administration services. PAI, EBS and Thomas Cooper are separate companies.

You can submit primary, secondary and corrected claims for both professional and institutional providers.

Superbill Claims

Submit CMS-1500 claims to us using the Superbill tool within My Insurance Manager. This tool is ideal for providers who want to submit primary claims for one date of service only.

Electronic Claims

If you prefer electronic claim submission, we also accept the HIPAA 4010A1 electronic format for both professional and institutional claims.
