BlueCross and BlueChoice® HealthPlan started the Physician Recognition Program (PRP) to promote good health and fight disease. As you know, heart disease and diabetes are among the leading health issues affecting South Carolinians. PRP is a voluntary program available to Preferred Blue® and BlueChoice HealthPlan network providers only.
How to Qualify
Network physicians who receive recognition from one of three programs listed below may receive $2,500 from both BlueCross and BlueChoice HealthPlan for a total of $5,000.
The physician is responsible for all costs of the application process. We have arranged for a 20% discount for the NCQA programs for eligible physicians who apply. Physicians must use the BlueCross BlueShield of South Carolina/BlueChoice HealthPlan NCQA Discount Form when making the application.
How to Apply
Please review the Terms & Conditions. If you have already met the qualifications, congratulations! Just mail the following information to both BlueCross and BlueChoice HealthPlan to get your award. The addresses are on the form.
- Physician Recognition Program Payment Request Form
- The letter of recognition or certification from the appropriate program
- W-9 Form
